Immigration Physical in Cary (I-693)

USCIS Immigration Physical Exam

Cary Medical Clinic offers the Immigration Physical Exam at our doctor’s office that’s conveniently located at our Cary, NC location. Dr. Neelu Agarwal, MD is designated as a Civil Surgeon for the U.S. Department of Homeland Security, U.S. Citizenship and Immigration Service (USCIS). A USCIS approved Civil Surgeon is required to perform the Immigration Physical as part of the entry and/or residency application process. This includes application for green card, citizenship or permanent residency in the United States. The medical exam form I-693 that is completed during the Immigration Physical is used to determine admissibility to the U.S. on medical grounds.

USCIS Immigration Physical Exam Requirements

The US immigration medical requirements are determined by the USCIS guidelines. The components of each medical examination vary based on the age of the applicant and whether the applicant can provide written immunization records / proof of vaccination. Exam Requirements:

An adult examination (15 years or older) typically consists of the following:

  1. A Physical Examination
  2. A blood test for tuberculosis
  3. A blood test for HIV, syphilis
  4. A urine test for gonorrhea
  5. Immunizations are dependent on age and medical history

A pediatric examination (children 14 years of age or younger) does not usually require blood testing and typically consists of the following:

  1. A physical examination
  2. A test for tuberculosis (2 years old and older)*
  3. A review and update of the pediatric immunizations

Tetanus/Diptheria Vaccine

Varicella (Chicken Pox) Vaccine

Seasonal Influenza (during flu season for certain individuals)

Childhood Immunizations (depends on age):

Rotavirus Vaccine

Measles, Mumps, Rubella Vaccine

Tetanus, Diptheria, Pertussis Vaccine

Polio Vaccine

Haemophilus Influenzae Type B Vaccine

Hepatitis B Vaccine

Hepatitis A Vaccine

Pneumococcal Vaccine

What to Bring:

  1. Picture identification such as a passport or other Federally approved documentation.
  2. Your medical records for any chronic or serious medical problem. If medical records are not in English please have them translated before presenting for Medical visit. Applicants who do not speak English will be provided with a translation service. Please let us know if this is required when the appointment is booked.
  3. Your immunization records, even childhood records, if available.
  4. Alien Registration number if you have one (starts with an “A”).
  5. Bring your I-693 medical form to the office. Please use the most recent copy of the form. The most recent form is available on the internet by following this link: Form I-693, Report of Medical Examination and Vaccination Record (uscis.gov).
